To assure that your Common Manual remains current, please record this document on your LPM/LPB Index, and retain it in Appendix E of your manual.
Effective May 19, 1998, the U.S. Department of Education (the Department) deactivated the following lenders for failure to file a Lender's Interest and Special Allowance Request and Report (ED Form 799) within the last two quarters:

816268 Coastland Federal Credit Union
816270 Post Office Employees Credit Union
822991 Monroe Telco Federal Credit Union
830823 Parish National Bank of St. Tammany
833339 Jefferson Savings Bank
FFELP loans may not be guaranteed for deactivated lenders, and LOSFA will not pay default claims for loans on which origination fees have not been paid. Lenders with questions about their deactivation should contact the Department at the following location:
