To assure that your Common Manual remains current, please record this document on your LPM/LPB index, and retain it in Appendix E of your manual.
A. Effective May 19, 1997, the U.S. Department of Education (the Department) deactivated the following lenders for failure to file a Lender's Interest and Special Allowance Request and Report (ED Form 799) within the last two quarters:

4th District LA DOTD Credit Union, #820640
Premier Bank, #823564FFELP loans may not be guaranteed for deactivated lenders, and LOSFA will not pay default claims for loans on which origination fees have not been paid. Lenders with questions about their deactivation should contact the U.S. Department of Education at the following location:
