By submitting this request, you are certifying that you are a designated representative of the district/school and that your district/school wishes to enter an agreement with LOSFA to receive FAFSA Completion Data through LOSFA's automated reporting system. Any questions concerning your request will be sent to the email address entered below. If LOSFA has no questions, your district/school will be added to our records and an Excel spreadsheet containing the requested data will be sent weekly to the email address entered below.
