To assure that your Common Manual remains current, please record this document on your LPM/LPB index, and retain it in Appendix E of your manual.

All promissory notes for LOSFA guaranteed loans will be mailed from Fishers, Indiana beginning May 1, 2000. Schools should continue to direct students to call us for duplicate notes. We can request them through our system so that no time will be lost in obtaining them. The notes will look identical to the ones currently printed for USAFunds guarantees, except that LOSFA’s name will now appear in the upper right hand corner of the form.

LOSFA also reminds you that all notes must be Master Promissory Notes beginning July 1, 2000.
