The U.S. Department of Education has approved new loan discharge/cancellation forms for the FFELP and the Direct Loan Program (see attached April 1999 Dear Guaranty Agency Director letter). The five separate application forms reflect requirements pertaining to discharge or cancellation of loans due to school closure, false certification of ability to benefit, false certification of disqualifying status, unauthorized signature or payment, and total and permanent disability.
These forms may be used immediately, however, use is mandatory when providing loan discharge or cancellation applications to borrowers on or after August 1, 1999. Other loan discharge or cancellation applications may still be processed after that date, but all new applications sent to borrowers on or after August 1, 1999 must use the enclosed forms.
