Scholarships & Grants (TOPS) Forms

You may request forms using any of the following methods:

  1. All of these forms are either available online or as downloads.  The downloadable forms are in .pdf format and you will need to use Adobe Acrobat Reader to read and print these forms on your printer.  If you don't have this software loaded on your PC you can get it for free from Adobe.  Follow their instructions carefully to download, install and configure your browser.  Once your browser is configured just click onto the "download" link next to your desired form or you can right-click onto the link and choose Save Target As....(using Microsoft Internet Explorer) to download the form to your hard drive (i.e. C:\ drive).  Keep in mind where you save the form so you can retrieve it later at your convenience.

You must complete the FAFSA Form in order to apply for the TOPS scholarship unless you meet the criteria for the TOPS on-line application. 		Online  
Application to Return from an Out-of-State College
Use this form if you wish to return to a Louisiana college from an Out-of-State College.		   Download
Request for Exception
Use this form to request an exception to the continuous or full time enrollment requirement.		   Download
Care of Grandparent(s) Questionnaire
Complete this form if you are requesting an exception to the TOPS continuous enrollment requirement based on the care of your grandparent or great grandparent.		   Download
     
Residency Affidavit
This form is a residency affidavit for students graduating from high school in 2006 and Thereafter.		   Download
Special Hurricane Residency Affidavit
This form is a special affidavit of residency for Hurricane "Displaced Students" Graduating High School During the 2005-06, 2006-07, 2007-08 or 2008-09 Academic Year. 		  Download

This form is an affidavit of residency for students who graduated from high school in 2004-05 Only.		   Download
